Home Care Assessment
The style of assessment depends upon the needs of the individual. For most people, a transition period is required, during which time staff from the service spend time at the person’s own home/hospital as well as their daycare placement or school.
Initially, the person will visit one of our bungalows for a few hours and then later for a longer period to become familiar with his/her new surroundings, staff and future peers. When a person moves into one of the bungalows a 12 week assessment period commences during which all new residents have a detailed skills assessment, a functional analysis of their challenging behaviors and a medication review. Additional assessments as recommended may include speech and language.
There is a mild assessment case review after 6 weeks and the assessment period concludes with a full case review after 12 weeks. This determines strengths and needs and sets goals for the future of the residential placement and involves the resident, his/her parents, care manager and any other professionals who are involved.
Review Process
An integral part of our care is constant evaluation and review. We plan a minimum of two reviews a year (more depending on need or request) based on a detailed assessment and individual service plans.
The key workers and link workers responsible have the task of gathering information, organizing dates and times, involving staff teams and families and spending as much time as possible with the resident, explaining the process and gaining his/her views. An action plan is drawn up during the review setting goals and designating people responsible for implementing action and a new program is formulated for the next period. The review meetings involve the resident as much as possible.
Individual Service Plans
Each bungalow operates our own in house care plans known as Individual Service Plans (ISP’s). These are seen as core documents, guiding our work with each resident. The ISP is an extended individual program that includes detailed background information, strengths and needs on all aspects of an individual’s life and details specific procedures to be used with an individual in these areas, including short and long term goals. The ISP is updated regularly and is co-ordinated by the resident’s key worker and link worker. The staff aims to identify the strengths and needs of service users and devise strategies that maximize their potential.
Client Advocacy
Ashfield Court promotes client self-advocacy. Key workers and Link Workers are assigned to individuals to support them with their personal needs and choices and to help them understand their rights as individuals.
Social Skills
Socialization and new life experiences are of paramount importance to an individual’s personal development. Skills addressed include basic social skills, shopping/monetary skills, environmental awareness, and road safety.
